Health Net Oregon vendor reports a Data Security Incident

We are sending this letter to inform you that on January 7, 2021, Health Net Health Plan of Oregon learned of an incident that resulted in a potential disclosure/breach of your Protected Health Information (PHI).

We determined that on December 15, 2020, a data retrieval company failed to properly secure your printed health information. The information included your name, contact information, Social Security number and health care information.

After we learned about this situation, we performed a thorough investigation.  The data retrieval company took appropriate personnel action and retrained its employees. The company provided us information about actions they took and has been cooperative in answering our questions.

We are notifying you about what happened so you are aware of it.  You may want to review your Explanation of Benefits documents. If you believe you have experienced identity theft, please report your concerns to law enforcement, including the Attorney General and the Federal Trade Commission.
